Congratulations on taking the next step in joining our program! Before your first day, we just need a few quick pieces of paperwork to get you officially set up. Please find your age category below to access your required forms and checklist.
Students Ages 15 and Under
We’re thrilled to have you! Please complete the required registration forms below with a parent or legal guardian before orientation.
Note for Parents/Guardians: Some forms require a parent or legal guardian signature. Please review and sign all documents before submitting.
Students Ages 16 and Up (Paid Participants)
Welcome aboard! Because your time with us is paid, you are officially being onboarded as a paid participant. In addition to completing your paperwork, you must bring two valid forms of identification to orientation so we can set up your payroll.

You must present two forms of identification on your first day to verify your eligibility to work. See the 16 & up packed for a clear list.
Form 1: Identity & Photo ID. Form 2: Work Authorization
Example 1: State-issued Driver’s License or IDSocial Security Card
Example 2: School ID with photoBirth Certificate
U.S. Passport (Counts as BOTH Form 1 & Form 2!) Permanent Resident Card
🚨 Important: Photos or photocopies of IDs cannot be accepted for legal verification. Please bring the original, physical documents to orientation. No IDs = delayed paycheck!
